The crash in Kentucky is a “cascade” type accident, what were the factors that caused this crash?

The cascading effect means that a certain “start point” of events rolled over to another event etc. which in and of themselves would not have caused this crash, but together, doomed this flight to disaster. Any ideas out there (especially from CRJ crew) as to what caused this crash. Could it have been the dreaded ” I am the best pilot in the world, even though below V2, I can make it fly” syndrome?

Who sings a song about being high on an airplane?
